Gather these documents:
- Your Coast Guard ID
- CGMA-53 Medical/Dental Loan Application
- CGMA-15 Personal Budget
- Medical/dental bills
- Doctor’s statement that the treatment is medically necessary
- Denials from TRICARE, private insurance, or other sources

What's Covered?
CGMA provides assistance for extraordinary, unexpected, or emergency situations for expenses not covered for by TRICARE, the Coast Guard, other government agencies, private insurance coverage (including TRICARE supplements), or other organizations.
- Client must demonstrate financial need.
- Expenses must be medically required, not elective.
- Expenses must be denied by TRICARE or private insurance and other resources must be exhausted.
Most assistance is by interest-free loan up to $6,000, but CGMA may consider a larger amount or a partial grant in extreme situations.
Examples
- Partial payment required before treatment
- Patient’s cost share
- Mental health and family counseling
- Purchase or rental of medical equipment
- Orthopedic shoes, hearing aids, glasses, artificial limbs, implants, orthopedic appliances, and braces
- Temporary rehabilitation care, nursing care, home care, or respite care
- Orthodontic treatment or other long-term dental care
- Travel, food, lodging, childcare, and necessities
